Safety Contract

This safety contract, when signed, indicates the Student's full understanding and agreement with the following safety procedures when the class is participating in science experiments in class.

{Student's Name}, hereafter referred to as "Student," agrees to abide by the following rules and restrictions:

• Student will wear protective glasses before beginning any experiments.
• Student will follow all instructions, written or verbal, given by the teacher.
• Student will not depart from the instructions in any way. The student will not create or participate in any experiments that deviate from the lesson plan.
• Student has been informed of the emergency wash stations and knows how and when to use them.
• Student will not eat or drink anything in the lab station.
• Student will keep {his/her} station clean and will tidy up after experiments.
• Student will not start experiments or handle equipment without permission from teacher.
• Student will use caution around heat and chemicals.
• Student will make sure that loose clothing or long hair is restrained in some way.

{Guardian's Name}, hereafter referred to as "Guardian," acknowledges the following:

• Guardian has read the above safety rules
• Guardian understands that if Student violates the above rules, he or she will be subjected to an office referral or {punishment.}
• Guardian agrees to encourage Student to abide by the above rules and to behave in a safe and respectful manner in the lab.
