Going Out Contract

    

I, {Teen Name}, agree to abide by the following rules concerning going out unsupervised by a parent or other adult figure:

I will keep my cell phone on me at all times. If my parent(s) call, I will answer immediately.

I will let my parents know the places I am attending and the people who are joining me before I leave.

If my plans change, I will call and let my parents know.

I will provide phone numbers and/or addresses for the locations I will be visiting.

I will ask my parents' permission before visiting new places or spending time with new people.

My curfew is: {Time}. At that time I will be home, by myself and inside the house.

If I violate these rules, I understand that the following punishments and/or revocations will apply:
