FIELD TRIP RELEASE

I, {Legal Guardian Name}, verify that I have read and agree to the following terms and conditions for my child, {Child Name}, to attend the field trip to {location}.

• I have explained to my child the importance of listening to teachers and chaperones, following rules, and behaving in a respectful manner while in public. I understand that if my child does not adhere to the rules, he or she will be required to stay at school for the duration of the trip.
• I understand that my child will be exposed to: {potential allergens, animals, etc.}. My child has an allergy to the following: {allergies}. My child will require the use of {equipment} for the duration of the field trip, which I will provide.
• I grant permission for my child to ride in a car, bus, or other approved vehicle to and from the field trip location. I further grant permission for my child to be in a vehicle driven by a licensed teacher, student, or chaperone. I hereby hold the drivers, students, chaperones, teachers, and administration harmless and waive the right to bring charges against any party in the event of injury, illness, loss/theft/damage of property, or death.
• If my child is licensed to drive him or herself and others, I grant permission for him or her to drive other students to and from the field trip location.
• I understand that if I do not sign this form, my child will be unable to attend this field trip.
