Daycare Contract

This contract is an agreement between {Daycare Provider} and {Parent} regarding the care services provided to {Child}. These services may change in the future, at which point a new agreement will be signed.

 

Payment

The rate for daycare is {insert rate and interval}. This rate is for {explain amount of service} on the following days and times: {list child's schedule}.

Payment is due {payment due date}. Late fees will be {late fees}. All fees are due before the child can attend daycare.

 

Medications, Illnesses, Diapers

This daycare is able to handle medications and administer them to your child. Special concerns, such as allergies, should be explained in detail in writing for the child's file. If your child is ill, you must keep the child home. If the child becomes ill at daycare, you will be notified immediately and must arrive to take your child as soon as possible. Diapers must be brought from home, and you will be billed for diapers used if yours run out.

 

Meals

Meals are provided by the daycare. Please do not bring snacks of your own unless discussed previously with daycare personnel. This is important because other children may have allergies.

 

Holidays

The daycare is closed on the following days: {list known holidays}. Additional closures may occur, and will be made known to you in writing. If for any reason an emergency closure occurs, you will be notified as soon as possible and will be directed to a backup daycare.
