Covid-19 Prom Contract

_______________________ (hereafter “Student”), hereby agrees to the following rules regarding the prom taking place on ________________ at _________________ High School (hereafter “School”):

• Prom will be held in {location} from {time} to {time}. No admission will be granted after {time}. Re-entry {is/is not} permitted.
• Prom is open to {age groups} and the guests of those attendees. The Student is permitted {number} guest(s) and may purchase tickets in advance from {contact info}. Guests must be cleared with {Name} no later than {date}, and guests must be {age restrictions}.
• Distance seating will be made available for social distancing purposes. Students are encouraged to maintain distance from those who are not in their immediate party or bubbles. Dancing must be at least {distance} apart.
• Temperature checks are required for entry. Students who are experiencing fever/chills, nausea, diarrhea, cough, congestions/running nose, loss of smell/taste, and muscle or body aches are required to stay home. Refunds will be available for those who are sick or quarantined.
• Masks must be worn over the nose and mouth at all time by all students and staff.
• Handwashing stations and hand sanitizer will be available throughout the premises.
• No alcohol or drugs of any kind are permitted on the premises. Students who are intoxicated will be refused entry and/or removed from the prom.
• The School will provide {coat check/photo station/etc.}. Food/drink {will/will not} be provided.
• Dancing should be limited to {restrictions}.
• All school rules are in effect while on the premises.
