COVID-19 Hotel Waiver of Liability

This agreement releases {Hotel} from all liability if I contract COVID-19 while staying at {Hotel}. By signing this agreement, I agree to hold {Hotel} entirely free from any liability, including financial responsibility for any medical attention I may require.

I understand and agree to abide by the following precautions put into place by {Hotel}. If I do not understand, I will ask for clarification before signing:

• I will wear a mask over my nose and mouth at all times while in shared spaces inside the hotel, including the lobby, elevators, stairs, hallways, and gift shop.
• I will stay at least {number} feet away from other patrons and staff at all times.
• I will not have access to {gym, pool, restaurant/bar, ballroom, conference room} due to {state/county/city} COVID-19 restrictions.

{Hotel} will provide masks for those who have forgotten theirs and hand sanitizer for general use. {Hotel} has created spacing and ventilation for maximum airflow. Staff will wear gloves and masks and will thoroughly disinfect and ventilate the rooms at least {number} hours before guests arrive. {Hotel} will disinfect high-traffic areas and high-touch surfaces every {amount of time}, including {cleaning procedure details}.

I understand that {Hotel} has put the aforementioned precautions into place to protect me and the other patrons. {Hotel} will enforce all aforementioned precautions in order to provide maximum safety for patrons and staff. However, I understand that the precautions do not remove the risk of contracting the coronavirus in the building.

By signing this waiver, I attest that I am not currently sick, that I have not had a fever or any COVID-19 symptoms in the last 10 days, that I have not received a positive COVID-19 test in the last 14 days, that I have not been exposed to someone with COVID-19 in the last 14 days, and that I have not traveled to a high-risk area in the last 14 days.

By signing below, I forfeit all rights to bring a suit against {Hotel} for damages, illness, or death resulting from contracting COVID-19.
