COVID-19 Family Agreement

The members of the ______________________ household (hereafter collectively "the Family") hereby agree to the following precautions during the COVID-19 pandemic:

• The Family will wear masks and maintain a distance of at least six feet from other people when outside, shopping, or running errands.
• The Family will not spend time indoors with people other than the members of the household or approved members of the Family's bubble. The current list of approved members of the bubble are limited to: {Names}
• The Family will participate in low-risk activities such as {grocery shopping, outdoor socialization, doctor appointments} provided that they do so while masked and socially distanced and wash their hands with soap and hot water for 20 seconds before and after each activity.
• The Family will all participate in isolating for 14 days if any member is exposed to someone who has tested positive for COVID-19 or is experiencing COVID-19 symptoms.
• If a member of the Family tests positive for COVID-19 or exhibits symptoms, they will isolate within the house as best they can. They will wear a mask and wash their hands before entering shared spaces and will wash hands after touching their face, coughing, or sneezing.

The Family will observe the precautions listed above until all members have received the vaccination and waited the appropriate amount of time according to health officials.
