Costume Rental Agreement

____________________________ (hereafter "Costume Shop") and ______________________ (hereafter "Renter") hereby agree to the following:

The Costume shop will reserve {description of costume} (hereafter "The Costume") in a size {size} from {date} to {date}. The Renter will rent the costume at a daily rate of {rate} plus a security deposit of {amount} for a total of {total cost}, due {date}.

The Renter may return the Costume early for a prorated refund of {rate}. The security deposit will be returned to the Renter provided the Costume is returned in proper condition no later than {date}.

For every week late that the Costume is not returned, a {fee amount} fee will apply, up to the replacement cost of the Costume.

The Renter may not alter the Costume for fitting or adornment purposes. This includes painting, cutting, dyeing and sewing. Tape, glue or any adhesive is not permitted. Any alteration to the Costume must be approved by the Costume Shop in writing prior to implementation.

The Costume Shop will provide the Renter with a costume that is clean and unblemished, free of both stains and odors. The Renter must return the Costume in the same state. A description of how to safely clean the Costume is included in {attachment}.

If the Costume is returned with odors, stains, dirt or unauthorized alterations, a cleaning/repair fee will apply, up to {percentage} of the rental cost. If the Costume is lost or damaged beyond repair, the Renter will be charged the full replacement cost of the Costume, {amount}.

The Renter may cancel the agreement up to {amount of time} before it begins. After that time, any cancellation will result in a {amount} restocking fee.
