Classroom Rules Contract

{Teacher Name} has created a learning environment that is safe, welcoming, and encouraging for all students. Students who cannot abide by the rules of this classroom will be asked to leave.

{Student's Name}, hereafter referred to as "Student," agrees to abide by the following rules and restrictions:

• Student will be punctual to each class. (More than {number} tardies will result in {punishment}).
• Student will attend all classes. If Student is unable to attend class, he/she must provide {written parental permission, doctor's note, etc.}. More than {number} unexcused absences will result in {punishment}.
• Student will listen attentively to all lectures, lessons, and material presented. When he/she has a question, he/she will wait for an appropriate moment before raising his/her hand.
• Student will address the Teacher and all fellow students courteously.
• Student will be mindful of his/her own and others' safety.
• Student will be respectful of other people's cultures, beliefs, religions, and identities. Hate speech will not be tolerated.
• Student will not plagiarize material for class assignments or tests. All material must be original and all sources must be sited.
• Student will come to class every day prepared for class. This includes bringing necessary books, assignments, writing utensils, and other tools.
• Student will TURN OFF and properly stow all cell phones and other electronic devices before coming into class. Any electronic device not turned off and put away will be confiscated by the Teacher and not returned until {time limit}.
• Student will keep his/her workspace tidy and clean up his/her messes.

{Guardian's Name}, hereafter referred to as "Guardian," acknowledges the following:

• Guardian has read the above rules and affirms that the Student has read it as well.
• Guardian understands that if the Student violates the above rules, he or she will be subjected to an office referral or {punishment} at the discretion of the school.
• Guardian agrees to encourage the Student to abide by the above rules and to behave in a safe and respectful manner at all times in the classroom.
