Sport Team Contract

Student Athlete Contract

This contract is hereby made between {Student's Name} (hereafter known as "Student"), {Coach's Name} (hereafter known as "Coach"), and {Parents' Names} (hereafter known as "Parents").

Student's placement on the {Team Name} Team is based on merit and performance in tryouts. Student will be part of this team on the condition that {he/she} maintains the skills and dedication required for this division.

Coach and {School Name} School will provide {equipment, location, etc.}. However, a {amount} fee is required for {uniforms, transportation, etc.}. If Student cannot afford the fees, scholarships are available.

Attendance and participation is mandatory. If Student is unable to attend a practice, game, or tournament, {he/she} must have a written excusal from a parent, teacher, or doctor. After one unexcused absence, {consequence}. More than {number} and Student will be expelled from the team.

Parents understand that away games in other cities may require overnight stays or return trips past {time}, but that Student will always be chaperoned and part of a safe group. Student is responsible for paying for {his/her} own food and lodging on long or overnight trips.

If Student is over the age of {age} and has a valid driver's license, {he/she} may transport {him/herself/others} to games.
