DECLARATION OF TRUST

Grantor appoints Trustee to manage the above-listed digital assets, and Trustee agrees to accept this appointment and shall be responsible for performing necessary duties related to the assets, outlined below.

• Access and manage social media and accounts
• Preserve or delete online content as follows: {explain wishes}

ACCESS

Passwords, encryption keys, two-factor authentication, and other information required to access accounts is located {location}.
